Geriatric Care Managers are specialists who assist older adults and their families to plan for and implement ways to allow for the greatest degree of independence, safety and comfort while meeting their needs.

SeniorLink, LLC services include:

Care Planning Assessment

  • Evaluate care needs, identify problems, review eligibility for resources available
  • Recommend a plan of care through a review of programs and service options available

Coordination Of Services

  • Screen, arrange and monitor home support (home healthcare, homemaker/companion aide, day programs, meals, respite care)
  • Assist with decisions and selection of appropriate level of care which pairs preferences with required individual needs (independent retirement communities, assisted living, aging in place environments, or skilled-care facilities)

Serve As A Link/ Liaison

  • Provide routine home or facility visits
  • Regular communication to keep family apprised of their loved one’s well-being and/or alert them to new concerns
  • Ongoing monitoring to promote continuity between various service providers
  • Arrange appointments and transportation; accompany as needed
  • Attend Care Plan Meetings, and confer with other service providers as needed
  • Offer appropriate referrals to legal, financial and healthcare professionals

How Services And Fees Are Determined

  • All services are determined solely by the client and family through a consultation with a SeniorLink, LLC Care Manager
  • Service Fees can either be billed on an hourly fee-for-service basis, or a package can be tailor created to meet any budget or specific needs
